Notice: This form may be completed by individuals who desire to report observations of certain wildlife species of interest to the DNR's Bureau of Wildlife Management. Completion of this form is voluntary. Information collected is used for wildlife monitoring, research, and management purposes and is not intended for any other purposes. Personally identifiable information collected on this form will be used to process your submission and may be used by DNR staff to contact you to follow up. Information may also be provided to requesters as required under Wisconsin's Public Records Law [ss. 19.31 - 19.39, Wis. Stats.].
Instructions: Fill out the form as completely as possible and attach photo or video files if available. Indicate the specific location of the observation by clicking the location on the map. When finished, click the submit button to complete the form. Observations of other rare, native plant and animal species may be submitted as part of Wisconsin's Natural Heritage Inventory (NHI) Program. Sick or dead wildlife should be reported to the DNR Wildlife Hotline by emailing DNRWildlifeSwitchboard@wisconsin.gov or calling 608-267-0866 and leaving a detailed message.
