Description of Incident/General Comments |
CAUSE DESC: STATED WHILE TESTING THE INTERCEPTOR ONBOARD THE USS LITTLE ROCK, APPROXIMATELY 10-15 GALLONS OF HYDRAULIC FLUID RELEASED INTO THE MENOMINEE RIVER. THE CAUSE OF THE RELEASE WAS DUE SOME TYPE OF EQUIPMENT MALFUNCTION.
11/6/16, 10:00 AM, OCSC PC CONTACTED MM AND SPOKE WITH AARON WALTERS. THEY HAVE CONTAINED THE RELEASE WITH AN ABSORBENT BOOM IN THE WATER. HE WILL FOLLOW UP WITH RICK JOSLIN ON MONDAY 11/7/16
11/8/2016 RRJ EMAILED AARON WALTERS ASKING FOR DOCUMENTATION REPORT FOR CLEANUP ACTIVITIES ASSOCIATED WITH THIS SPILL. AARON WILL ATTEMPT TO GET INFO TO RRJ SOON.
