Description of Incident/General Comments
10.27.2021 - 11:04 - JP LEFT A MESSAGE WITH MARK DARBY. MARK CALLED BACK, DISCUSSED SPILL OVER SKYPE. WERE TESTING THE LINE AND USED DIESEL TO PUSH PROBE. DEBRIS MAY HAVE BLOCKED VACUUM HOSE, WHICH LED TO SPILL OUT OF THE CLEAN STORAGE TANK. APPROXIMATELY 25 GALLONS OF DIESEL WERE SPILLED. HAND DUG IMPACTED SOIL, DUG APPROXIMATELY 10-14 DRUMS. OVER DUG, BUT STILL COLLECTED CONFIRMATION SAMPLES. EXCAVATION COVERED WITH ABSORBENT PADS PENDING LAB RESULTS. 12.9.2021 - JP TEXT MATT TURNER FOR AN UPDATE. JP RECEIVED FINAL REPORT ON DECEMBER 10.

CLEANING THE LINE OF DIESEL FUEL AND THE FIRST BOX WAS NOT FEEDING INTO THE SECONDARY. THIS CAUSED THE FUEL TO COME OUT OF THE RELEASE VALVE.

THE GRAVEL ACCESS ROAD WAS IMPACTED BUT NO WATER WAYS
Cleanup Efforts at Time Incident Reported
OIL DRY APPLIED AND AFFECTED GROUND SCOOPED INTO 55 GALLON BARREL. GROUND WILL BE TESTED TOMORROW AS WELL. A PID WAS USED TO DETERMINE THE INITIAL EXTENT OF THE IMPACT TO SOIL. A 10' BY 25' BY 1.5' DEEP EXCAVTION WAS COMPLETED. FOUR CONFIRMATION SAMPLES WERE COLLECTED. A TOTAL OF 28 TONS OF SOIL WAS DISPOSED OF AT SHAMROCK LANDFILL.
